Parent/Guardian Agreement
To protect our players, coaches, volunteers and other Upper Perk Wrestling families, I agree to keep my child at home if he/she has any of the following:
- Fever (temperature of 99.4 degrees or more)
- Shortness of breath or difficulty breathing
- Cough
- Fatigue (not due to physical activity)
- Sore Throat
- Congestion or runny nose (non allergy)
- Muscle Pain/Joint Pain
- Nausea or vomiting
- Headache
- Diarrhea
- New loss of taste or smell
If my child has any of these signs of COVID-19, I will not send him/her back to an Upper Perk Wrestling activity until:
My child has been tested negative for COVID-19 and is otherwise well enough to go back to Upper Perk Wrestling activities OR a healthcare provider has seen my child and documented a reason for the symptoms other than COVID-19.
OR All of the following are TRUE:
- At least 10 days since the start of symptoms AND
- Fever free without anti-fever medications for 3 days AND
- Symptom free for at least 72 hours
If my child is diagnosed with COVID-19, I will not send him/her back to any Upper Perk Wrestling activities until the following and immediately notify Upper Perk Wrestling co-chairman Bryan Moyer at 267-884-4311.
- It has been at least 10 days since my child first had symptoms AND
- My child has had no fever without the aid of anti-fever medication for 72 hours AND
- My child is symptom free for a minimum of 72 hours
- Cleared with provided paperwork by a doctor
If someone in my household is diagnosed with COVID 19 or my child is exposed to COVID-19, I will keep him/her home for a minimum of 14 days and immediately notify Upper Perk Wrestling co-chairman Bryan Moyer at 267-884-4311.
If someone in my household develops a new cough, shortness of breath or two of the following: sore throat, chills, muscle/joint pain, headache, new loss of taste or smell, I will get that person tested for COVID-19. If that person tests positive, I will keep my child(ren) home for a minimum of 14 days and immediately notify Upper Perk Wrestling co-chairman Bryan Moyer at 267-884-4311.
